New GP at Rhyddings Surgery

We welcome Dr Atiya Raza who will be working with us for the next 6 months.  She is part-time with the Practice and will be in surgery Wednesday morning, Thursday morning and afternoon and Friday afternoon.

Extended Access

Cornerstone Practice and Healthcare is a member of Blackburn with Darwen’s Local Primary Care GP Federation. Local Primary Care Federation runs a GP extended access service that provides additional appointments at four neighborhood spoke surgeries being Darwen Healthcare, Family Practice in Barbara Castle Way Health Centre, Cornerstone Practice Shadsworth Surgery and Little Harwood. Patients can be booked into one of the four spoke surgeries via the practice receptionists accordingly. Please contact our reception for more information.

Electronic Prescription Services

We are able to send prescriptions electronically to a nominated pharmacy. (Please note that certain prescriptions cannot be sent electronically).

Important notice for new parents

Congratulations on the birth of your new baby! From the age of two months, your baby will be due to receive very important immunisations. You will receive an invite through the post from Child Health, asking you to make an appointment at the surgery for the baby to receive the vaccinations.
Please note that we will require parental consent at each visit for the vaccinations due that day. In order to protect our patients and our staff please be aware that it is our Practice policy not to vaccinate children for whom we have not received parental consent. If another family member will be bringing your baby for the appointment, we may need to contact you via telephone at the time of the visit to confirm consent has been given. Alternatively, you can choose to send a signed & dated note authorising for the vaccinations to be given in your absence.
Please accept our apologies if this policy causes any inconvenience for you.

Named GP for all patients

From 1st April 2015 all practices in England are contractually required to allocate every patient a named, accountable GP. Prior to this new arrangement, the practice already operated a system where patients, at the point of registration, are given a ‘usual GP’. In most cases, this is the doctor whom you would usually see at the practice. To that end, all patients have been allocated a named GP in accordance with their usual GP.
Your named GP will have overall responsibility for the care and support that our practice provides to you. However, please be aware that this does not prevent you from seeing any other GP in the practice as you are currently offered. Also, it may be the case that your named GP is not always available to see you, in which case another GP will be offered.
If you would like to find out who your named GP is then please do not hesitate to contact the surgery.

Changes to health review invitations

If you currently attend the surgery for reviews of a long term condition such as COPD, dementia or hypertension you may notice that your next invitation will include an arranged appointment for you. This is to improve the quality of the care you receive from our nursing staff by ensuring they have plenty of time to review your health conditions. It will also hopefully save you a telephone call to the surgery. However, if when you receive an invitation, the appointment arranged is not at a convenient time, then please do not hesitate to contact the reception team who will happily rearrange it for you.

Here at Lambeth Street we are offering those eligible the opportunity to come to our drop in clinic on Saturday 17th October, any time between 10am and 1pm. There is no need to make a prior appointment, if you are registered at Lambeth Street surgery all you have to do is just turn up and our team will ensure you receive your vaccination!

If you cannot attend this clinic, then please contact the reception team who will be happy to arrange a convenient appointment for you.

Likewise, if you have already received your vaccination elsewhere, or do not wish to have a vaccination this year, then please let us know. This will ensure your medical record is kept up-to-date.
