About Cornerstone Practice & Health Care

Cornerstone Practice is an NHS General Practice which consists of four surgeries at: 

Shadsworth, Blackburn. Rhyddings, Oswaldtwistle, Lambeth Street, Blackburn, and Bentham Road Health Centre, Mill Hill, Blackburn. (Bentham Road is run by Cornerstone Healthcare Community Interest Company). 

New patients  

We accept anyone who lives in our practice area as new patients.  

How to Register 

You can register by completing a registration form (called a GMS1 form) which can be obtained from the practice or downloaded here: GMS1 – GOV.UK (www.gov.uk) 

The completed form needs handing in at the practice. 

When you register, we will ask you to make an appointment to see either the practice nurse or healthcare assistant for a new patient health check. This will make sure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up to date.  

The Cornerstone Practice is a Christian Practice, founded by Christian Doctors and staff.
We will never impose our beliefs on people, but will treat people equally and will respect them as unique and important individuals.

The practice was started on 4 July 1988 to provide excellent whole person care for the Shadsworth area.
The Rhyddings surgery opened in 1996 in rented accommodation, and moved to its current building on the 1st May 2003.
on 29th August 2006, the Mayor & Lady Mayoress of Blackburn opened our surgery at Lambeth Street.
In the Summer of 2009 Cornerstone Signed the contract for Cornerstone Healthcare to be ran from Bentham Road Health Centre.

Our aim is to provide excellent whole person care in a relaxed, friendly atmosphere with a good training environment.
